🩺 Post-Resuscitation Care (UK/ALS 2021)
- 🫁 Airway & Breathing
- Ensure airway patency; consider advanced airway (ETT) if needed.
- Give O₂ to maintain SpO₂ 94–98% (avoid hyperoxia).
- Monitor for distress or impending respiratory failure.
- ❤️ Circulation
- Continuous ECG monitoring; repeat 12-lead ECG.
- Blood pressure + rhythm monitoring; consider CXR & echo.
- Optimize cardiac output → fluids, diuretics, inotropes, vasodilators as appropriate.
- If hypotensive despite fluids → start vasopressors (e.g. norepinephrine).
- 🌡️ Temperature Management
- Targeted temperature management (TTM) 32–36 °C for ≥24 h.
- Cooling blankets, ice packs, intravascular devices; monitor core temperature closely.
- Prevent rebound hyperthermia.
- 🧠 Neurological Care
- Assess GCS; use sedation scoring if intubated.
- Monitor for seizures; EEG if available.
- Early neurology consult if poor recovery or seizures.
- 🧪 Diagnostic Testing
- 12-lead ECG for STEMI/ACS ➝ urgent PCI if indicated.
- Bloods: ABG, electrolytes, cardiac biomarkers, lactate.
- Imaging: CXR for tube/line check + pulmonary oedema; CT if intracranial pathology suspected.
💡 Key Pearls:
– Post-ROSC care is about organ support + cause correction.
– Maintain MAP ≥65 mmHg, SpO₂ 94–98%, TTM 32–36 °C.
– Always search for the underlying cause: 4 Hs + 4 Ts.
