๐ฌ About
- ๐ฆ Trichomoniasis is caused by the protozoan parasite Trichomonas vaginalis.
- โค๏ธโ๐ฅ It is a sexually transmitted infection (STI) affecting the urogenital tract.
- ๐ฉ In women, infection usually involves the vagina and often the urethra; ๐จ in men, it usually affects the urethra.
- ๐ถ Many infections are asymptomatic, especially in men, so transmission may occur unknowingly.
- ๐ Concurrent STIs are common, so full STI screening is recommended.
๐งฌ Aetiology / Transmission
- ๐ค In adults, transmission is almost exclusively through sexual intercourse.
- โ ๏ธ Risk factors include multiple sexual partners, previous STIs, and lack of barrier protection.
- ๐ง Fomite transmission is not a usual route and should not be emphasised.
๐ฉบ Clinical Features
- ๐จ In men:
- ๐ถ Often asymptomatic.
- ๐ฅ Dysuria or urethral irritation.
- ๐ง Urethral discharge.
- ๐ฉ In women:
- ๐ฅ Vulval irritation, soreness, or itching.
- ๐ฝ Dysuria.
- ๐จ Vaginal discharge, which may be offensive; classical frothy yellow discharge occurs but is not always present.
- โค๏ธโ๐ฉน Dyspareunia may occur.
๐ Investigations
- ๐งฌ NAAT is the preferred diagnostic test.
- ๐ฌ Wet mount microscopy may identify motile trichomonads but is less sensitive.
- ๐ฉธ Offer full STI screening (for example HIV, gonorrhoea, chlamydia, syphilis as appropriate).
- ๐ฅ Referral to a sexual health / GUM clinic is strongly recommended for diagnosis confirmation, partner notification, and management.
๐ Management
- โ
First-line: Metronidazole 400 mg twice daily for 7 days.
- ๐ Alternative: Metronidazole 2 g single dose if adherence is a concern.
- ๐คฐ In pregnancy or breastfeeding, avoid the 2 g single-dose regimen; seek specialist sexual health advice.
- ๐ฉโโค๏ธโ๐จ Treat the current sexual partner(s) and any partner(s) from the previous 4 weeks.
- ๐ซ Avoid sex for at least 1 week and until the person and partner(s) have completed treatment.
- ๐ท Avoid alcohol with metronidazole.
- โ Clotrimazole may help vaginal symptoms but is not curative.
โ ๏ธ Complications
- ๐งฉ Increased risk of transmission or acquisition of other STIs.
- ๐คฐ Associated with adverse pregnancy outcomes including preterm delivery and low birth weight.
- ๐ Persistent symptoms may reflect reinfection, poor adherence, resistance, or an alternative diagnosis.
๐ก๏ธ Prevention
- ๐งท Condom use reduces risk.
- ๐ข Partner notification and treatment are essential.
- ๐ Consider repeat assessment if symptoms persist or recur.
