A 78-year-old male former furniture shop owner presents with weight loss, fatigue and weakness. When you examined him you find marked proximal weakness and finds it difficult to sit to stand and walk upstairs or raising his arms above his head. He has dilated pupils. His reflexes are reduced at rest and appear to increase after exercise. He has a waddling gait.
You look through his results and you can see he has a visible hilar mass on his CXR and this has been confirmed by a CT Thorax. He has a 60-pack year of smoking.
He has been seen by the respiratory team who have done a Bronchoscopy has been done and biopsies were taken which confirms this to be a small cell cancer.
